Note: Forms postmarked after Wednesday, February 15, 2006, faxed
after 5:00 p.m. ET on February 15, or not paid by February 15 are payable at the
regular/On-site registration rate.

Fees. Fees include all sessions, coffee breaks, reception and luncheon,
plus conference materials. Please include payment with your registration form. NACUA does
not accept telephone registrations. Fax registrations must include Visa or MasterCard
payments.
Cancellation Policy. Requests for refunds (less a $50 processing fee)
will be honored only if NACUA receives written notification of the cancellation via e-mail
at mmcmillan@nacua.org or via
fax at (202) 296-8379 by 5:00 p.m. ET, February 24, 2006.
NACUA will NOT issue refunds after this date.
