like NACUA to apply on my behalf for CLE Credit. How do I do this? |

Attorneys interested in having
NACUA apply for their CLE credit must complete two forms, the Certificate of Attendance
form and the Attendance Record, which are included in the materials. Please return both
forms along with payment of $5, per person per state requested, to NACUA by mail or fax
(202-296-8379). Depending on the state's rules, NACUA will sign the form to verify your
attendance at the seminar and will either process your credit with the MCLE record-keeping
authority in the state(s) you provided or return the signed form to you for your records.Please note, if you are
requesting credit in more than one state you must fill out one form per state.

Attorneys in some states need a specialized
Certificate of Attendance form to submit to their state's MCLE record-keeping authority.
Once the Attendance Records are received, NACUA will send out specialized forms to be
distributed to participating attorneys. Please contact Meredith McMillan and let her know how many forms you need. The states affected are: KY, LA, TX, VA.
